Trial Class Agreement : Health, Safety & Media

PLEASE READ CAREFULLY. THIS DOCUMENT AFFECTS YOUR RIGHTS.

I am a participating adult, or the parent/guardian of a participating child, taking part in a free trial or drop-in session at Gracie Barra Harrogate, Gracie Barra Apperley Bridge, or Gracie Barra York.

My (or my child's) participation is voluntary and I understand that Brazilian Jiu-Jitsu is a physical activity involving contact and inherent risks. I acknowledge that Gracie Barra (including its staff and students) is not liable for any risks or injuries sustained as a result of training.

It is my personal responsibility to inform instructors and staff, in person at the start of the session, of any health conditions, prior injuries, or other reasons that might affect my (or my child's) ability to participate in usual exercise activities.

I acknowledge that CCTV surveillance is in operation on Gracie Barra premises , and my (or my child's) image may be captured.

I understand that photos and videos may be taken during classes for social media and promotional use. If I do not wish for my child to be photographed or videoed, I will inform the staff at the start of the session.
