INTERNATIONAL TRAINING CENTER LIABILITY WAIVER & RELEASE AGREEMENT

By signing this form, I acknowledge that I have been advised not to participate in any physical activity if I have any doubts or uncertainties about my current medical condition. I understand that I should consult a medical professional before engaging in any physical training program.

I recognize that the activities at the International Training Center are physically and mentally demanding, requiring extreme exertion and carrying the risk of injury or even death. I am responsible for listening to my body and stopping any activity if I feel unable to continue. I hereby certify that I am in good health and do not have any heart condition or other medical ailment that could be aggravated by participation in these activities. I understand and accept the risks involved and confirm that I am physically able to participate. Additionally, I agree to follow all rules, regulations, and instructions provided by instructors or assistant instructors at the International Training Center.

Furthermore, I, on behalf of myself, my heirs, personal representatives, executors, and administrators, hereby waive, release, discharge, and agree not to sue International Training Center, its owners, operators, officials, supervisors, officers, participants, agents, instructors, employees, or affiliates from any and all claims, liabilities, expenses, damages, injuries, or losses, including those resulting from my participation in martial arts classes, fitness training, or seminars. I agree to indemnify and hold harmless the aforementioned parties from any loss, injury, damage, claim, or liability.

I confirm that I either have insurance coverage for any injuries I may sustain or have chosen to participate without insurance, fully assuming all risks associated with bodily injury, death, and property damage. I accept full responsibility for any and all known and unknown risks related to my participation in these activities.

Additionally, I grant permission to the International Training Center to use any photographs or audiovisual recordings taken of me or my child during classes or events for promotional purposes. I understand that I will not receive compensation for the use of these images.

Membership & Payment Policy:

Our electronic payment system operates on an automatic monthly billing cycle. A minimum of 30 days' notice is required to process a leave of absence, membership suspension, or cancellation. Membership dues are non-refundable once charged for the month. Exceptions for suspending dues due to injury or illness will be considered on a case-by-case basis. In such instances, the member must notify the International Training Center immediately.

Acknowledgment for Minors:

I hereby certify that I am at least 18 years of age. If I am registering a membership for my child or minor in my custody, I acknowledge that by signing, I accept all terms and conditions of this waiver on behalf of my child or minor in my custody.
